A test that identifies whether a specific disc is the source of pain — and the only test that asks the disc directly.
What it is
Contrast is injected into a disc under pressure while you report what you feel. A positive result reproduces your familiar pain at one level and not at adjacent control levels. It is used selectively, mainly before surgical decisions.
What we use it for
- Deciding whether a specific disc is generating pain
- Planning before fusion or disc replacement
- Where imaging shows several abnormal discs and only one is symptomatic
What to expect
- You stay awake and responsive — your feedback is the test.
- Under fluoroscopy, 30 to 45 minutes.
- Increased pain for a few days afterwards is common.
- A driver is required.
Risks
Every procedure carries a small risk of bleeding, infection, an allergic reaction, or a temporary increase in pain. Steroid can raise blood sugar for several days, which matters if you are diabetic. Serious complications are rare, and the specific risks for your procedure and your health will be discussed before you consent to anything.

Common questions
Why do I have to be awake?
Because the result is whether the injection reproduces your usual pain. Sedation heavy enough to remove that removes the test.
