Burning feet at night

Burning Feet at Night

Feet that burn once you are in bed, and settle when you get up and walk, point to a nerve problem, not a blood flow problem. That difference decides everything that follows.

The burning that arrives at night while the same foot feels numb by day is characteristic, not paradoxical.

Why do my feet burn more at night?

Three things come together after you lie down. The day’s distractions are gone, so the signal now has your full attention. Skin gets warmer under the covers, and damaged small nerve fibers fire more when warm. And with the legs flat, no change of position brings relief.

So the burning really is worse at night, not just easier to notice. That is why patients describe pacing the kitchen at 3 a.m. and finding it helps.

Is it a nerve problem or a circulation problem?

Nerve pain is worse at rest and eased by movement. Circulation pain is worse with movement and eased by rest. That one contrast splits two conditions that need very different specialists.

  • Burning, tingling, electric, worse in bed, better walking — small-fiber neuropathy.
  • Cramping calf pain on walking a predictable distance, relieved by standing still — arterial disease. That is claudication. It needs a vascular (blood vessel) check, not a pain clinic.
  • Both together is common in diabetes. That is why the exam checks pulses as well as feeling.

If your feet are also cold, pale, hairless or slow to heal, that points toward blood flow, and it is urgent. See neuropathy or a circulation problem.

How do I stop my feet from burning at night?

  • Keep the feet cool. A cradle or a folded blanket that lifts the covers off them helps more than almost anything else, because warmth drives the firing.
  • Do not use hot water. A hot soak feels good for a moment. It is dangerous on a foot that cannot feel well, because scalds often lead to serious injury. Test water with your elbow, not your foot.
  • Get up and walk when it peaks. Movement really does calm small-fiber firing. Lying still and putting up with it does not.
  • Loose bedding and cotton socks if the sheet itself hurts. Allodynia (pain from light touch) is a nerve sign worth reporting.
  • Check the timing against meals. Patients often find the worst nights follow the biggest blood sugar spikes after meals. That is useful to know.

Never use a heating pad or hot water bottle on numb feet. If your feet have less protective feeling, you will not feel a burn forming. This is one of the most common causes of serious foot injury in diabetes, and it is fully preventable.

What causes burning feet at night?

Most often diabetic small-fiber neuropathy, including in people with prediabetes who have been told their sugar is fine. But burning feet also come from B12 deficiency, thyroid disease, alcohol, chemotherapy and several medications. Those are worth finding because they can be corrected.

When only the small fibers are damaged, standard nerve conduction studies can be fully normal. That is a known blind spot, not good news — see small fiber neuropathy.

Common questions

Which vitamin deficiency causes burning feet?

B12 deficiency. It causes exactly the burning described on this page. It can be corrected, so it is worth finding. Metformin, a common diabetes drug, can drain B12 over years. Thyroid disease, alcohol, chemotherapy and several medications also cause burning feet. So the exam looks for all of them, rather than assuming diabetes is the only cause.

Why does my bedsheet hurt my feet?

Pain from light touch is called allodynia. It is a nerve sign worth reporting. Loose bedding and cotton socks help when the sheet itself hurts. A cradle or a folded blanket that lifts the covers off your feet helps more than almost anything else, because warmth under the covers makes damaged small fibers fire more.

Does blood sugar affect burning feet at night?

Often, yes. Patients often find their worst nights follow the biggest blood sugar spikes after meals. So it helps to match bad nights against meals. Diabetic small-fiber neuropathy is the most common cause of night burning. That includes people with prediabetes who have been told their sugar is fine.

Should I soak my feet in hot water?

No. It gives brief relief and risks a scald you may not feel. Cool is safer and works better for burning.

Diabetic Foot Care and Preventing Ulcers covers protecting a foot with reduced sensation.

Why does walking help?

Movement calms small-fiber firing and gives the nerves other signals to carry. It is a real body effect, not just distraction.

That is the subject of Neuropathy or a Circulation Problem?

My nerve test was normal. Does that rule out neuropathy?

No. Standard nerve conduction studies measure large fibers. Pure small-fiber neuropathy usually looks normal on them.

Small Fiber Neuropathy explains why the standard test misses it and how it is found.

Could this be my diabetes medication?

Metformin can drain B12 over years, and B12 deficiency causes exactly this. It is worth testing — see our page on metformin and B12.

Is burning at night an emergency?

Not usually. Cold, pale feet, rest pain in the toes, or a wound that is not healing are urgent and point to blood flow.

Night-time burning and daytime unsteadiness often travel together. Balance and fall-risk assessment measures what the numbness is costing you in balance.

Alcohol improves the next hour and worsens the next month.
