- Type 1 represents inflammation and oedema in the bone marrow. It is the type most consistently associated with pain.
- Type 2 represents fatty replacement of the marrow and is generally more stable.
- Type 3 represents sclerosis — hardened bone.
- Types can convert into one another over time, and Type 1 in particular can settle.
What it means for you
Type 1 changes at a level matching your pain are one of the more meaningful findings on a spine MRI, and they point toward the vertebral endplate as a pain source rather than the disc itself. See vertebrogenic pain.
Remember the general rule. Findings like this one are common in people with no pain at all, and become steadily more common with age. A finding on a report is not a diagnosis until someone has matched it to your symptoms and your examination. See understanding your MRI report.

Are Modic changes an infection?
Type 1 changes look similar to early infection on imaging, which is why the distinction is taken seriously where there is fever or a relevant history. In the ordinary case it is a degenerative finding.
