Quad Set Exercise for Knee Pain

A quad set is a first exercise for a hurt knee: with your leg straight on a rolled towel, tighten your thigh so the knee presses down into the towel, hold 5 seconds, and repeat 10 times.

Light figure: starting position. Dark figure: end position. Arrow: the direction you move.

Not every exercise is for every patient. The ones meant for you carry a check mark on the home sheet your physician fills out; stick to those, morning and evening, every day, unless the sheet gives you a different schedule.

What are the steps for the thigh squeeze (quad set)?

  1. Get set: Sit or lie down with the hurt leg out straight. Tuck a rolled-up towel beneath the knee.
  2. Move: Firm up your front thigh muscle until the towel gets squashed under your knee.
  3. How many: Hold 5 seconds. Do 10.

How does the thigh squeeze (quad set) help you recover?

The big thigh muscle switches off quickly after a knee is hurt, such as when a knee hits the dashboard or twists in a fall. A weak thigh lets the knee buckle. The squeeze wakes it back up while the joint stays still.

Our page on knee pain explains how we sort out what was hurt in the knee.

Which mistakes should you watch for?

  • Holding your breath.
  • Lifting your heel instead of pushing the knee down.
  • Letting go early. Count out the full 5 seconds.

When should you stop and call us?

Feeling a stretch, a bit of ache, or next-day stiffness is expected and fine. Stop the exercise and call us if you notice any of these:

  • Sharp or stabbing pain
  • Pain that jumps 2 or more points above your usual and lasts over an hour
  • New numbness, tingling or weakness
  • Dizziness or a change in your vision

Call 911 right away for chest pain, fainting or trouble breathing.

Why does your exercise log matter?

Each time you finish your exercises, tick the box on your log, and jot your pain number down once a day. The log rides along to every visit. Your technician adds up your days, has you demonstrate one exercise so form can be checked, and enters that tally in the visit note, right beside the treatments.

What to remember

  1. Press the knee down into the towel.
  2. Hold the whole five seconds.
  3. Keep breathing.

Show us you have it: Do a quad set while we put a hand on your thigh to check that the muscle tightens. If any part is unclear, ask us to go over it again; there is no limit on that.
